Step 2: Map the Service Change

Identify the benefits of change, drivers and barriers.

Checklist

Goals and Benefits of Change

  • What are the drivers behind the service change?
  • What are the costs and outcomes under current models?
  • What are the intended benefits from the service change?
  • How will the change be effectively monitored?

Current Baseline Summary

  • What are the current costs and outcomes under the current models?
  • What performance indicators or service agreements are in place? E.g. waiting times, referral timeframes, service user outcome measures.
  • How and where are services currently being delivered? Do they need to change?
  • What performance data is available?
  • What data has been used and what assumptions have been made regarding the data?

 Drivers and Constraints 

  • What are the forces that support the service change?
  • What are the forces that hamper the change?

Option Appraisal

  • What different scenarios for service change have been considered?
  • Which is your preferred option and why?
  • DoMs there need to be a staggered approach to meet the optimum model?

Working models

  • Does the preferred model deliver the described benefits more effectively than other models?
  • Or is the model simply more likely to be achievable given the anticipated constraints?
  • Is there a clear and shared understanding of the future service configuration
